What is PREFERRED Access?

Home Health and Hospice of Kansas understands that we all want to have a good quality of life and know that there may be a time when you need more help. PREFERRED Access allows you to preplan for Home Health Care, Palliative Care, or Hospice Care when you become hospitalized or need more specialized care. This may include but is not limited to:

  • Physical, Occupational, or Speech Therapy
  • IV Therapy
  • Wound Care
  • Medication Management
  • Disease Education
  • Telehealth
  • Skilled Nursing, Home Health Aide, Chaplains, and Social Work
  • We Honor Veterans Program
  • Volunteers

To achieve care security simply complete the form below. A representative from Home Health & Hospice of Kansas will contact you about the program and other benefits available to you. Your information is secure. A Home Health & Hospice of Kansas representative will coordinate your care needs directly with your physician and hospital discharge planner if needed. There is no cost to you for this service.
