Hospice is a service provided to a patient to assist with care when they have an illness that could result in death six months or less if the disease follows its course. Many conditions can lead to death; this may include but not limited to cancer, congestive heart failure, chronic obstructive pulmonary disease, end-stage dementia/Alzheimer's disease, and many others.

  • Continuous Care Requiring Intense Intervention - eligibility required
  • Available 24 Hours, 7 Days a Week
  • Bereavement Program offered for 13 months after the death
